Checklist – Guide to filing a petition
Print this checklist to use as a guide to the steps necessary to have a Confidential Intermediary (CI) appointed by the court.
Don’t hesitate to call us if you have questions about any part of the process. Confidential Intermediary Service of Illinois (CISI) staff is available to help you by telephone (847-298-9096, ext 29) or in the office by appointment.
File with the Illinois Adoption Registry
Illinois law requires that you register with the Illinois Adoption Registry and Medical Information Exchange before submitting a petition requesting the appointment of a Confidential Intermediary.
How to submit the petition to the Court (for Courts throughout Illinois)
During the past 10 years, most people using the CI program have successfully submitted a petition to the Court without the help of an attorney. This section provides step-by-step instructions for completing forms to petition the Court. While CISI cannot provide legal advice, program staff can answer questions about the forms and help you through the process.
If you prefer to file with the assistance of an attorney, your local Bar Association may be able to refer you to an attorney familiar with the Confidential Intermediary program.
